Information you must not send
Medical information cannot be used by this site to arrange care. Submit necessary medical records directly to a facility using the secure method it specifies.
Do not send patient names, medical history, test results, prescriptions, insurance documents, referral letters, passport numbers, or other personal or medical information.
What to include
- Listing page URL
- Item that needs correction
- Proposed correct information
- A supporting public URL from the facility or another primary source
- Relationship to the facility (optional)
How requests are reviewed
We compare requests with public sources. We cannot guarantee a listing change or individual reply. If a supporting source cannot be verified, the item may remain “not confirmed”.
